Care Coordination

Effective two-way communication between primary care and dental clinicians is critical. Patients who are particularly in need of care coordination include those who are:

  • Medically complex
  • Taking anticoagulants
  • Require antibiotic prophylaxis

Primary care clinicians should:

  • Actively address oral issues and educate patients about oral care.
  • Provide dental consultants with written diagnosis and treatment recommendations.
  • Consider using a standardized communication form.
